I've grenaded my share of u-joints in my time, and every instance was as a result of the same circumstances... sudden shock of torque from a neutral-drop (automatic) or a high-rev clutch-drop. A shock like that through the drive train puts alot of stress on everything, and the weakest link (u-joints) will typically buy it before anything else. Bad design? No... just the way it is I'm afraid. I even managed to destroy a rear differential yoke (made by Spicer, one of the best in the world). An exploding u-joint's schrapnel also carries a high risk of destroying other components as well.
